The unilateral swollen mass in the rear of the hind legs below the vulva that felts hard when palpated is a testicle in a case of hermaphroditism.
Hermaphroditism is a condition of genetic origin in which the sexual organs have characteristics of both sexes. In this case, the second pig on the left has a vulva and one testicle down.
